You need to consider how you reference. There should be discussion and analysis embedded into your writing with referencing to support your discussion, rather then adding a reference at the end of a paragraph.

Also, there should not be paragraphs without any referencing when writing at level 5. Your reference list should be in alphabetical order Sometimes you refer to your patient as Sheila and sometimes as Mrs Reid.

Be consistent with one or the other There are limited references that relate to person centred care.

More references in relation to this would demonstrate that you are aligning your writing to the evidence base of PCC There are some good principles in your essay that link in well to PCC – you need to ensure you incorporate referencing to support these principles.
